I click lang ang attach files at puwede mo nang picturan ang mga requirements gaya ng ID, Reseta, Authorization Letter at Clinical Abstract galing sa doctor gamit ang iyong cellphone). *IMPORTANTE PO NA MA-ISAMA LAHAT NG KAILANGAN NA REQUIREMENTS SA IYONG MGA ATTACHMENTS tulad ng ID, Reseta, Authorization Letter (kung hindi ang pasyente ang may request), at Clinical Abstract galing sa Doktor.
