      Ang bahagi na ito ay puwedeng gamitin sa pag hingi ng Quotation ng presyo ng gamot na maaring gamitin sa PCSO o  sa DSWD. 

    Ang pag proseso ng request ay maaring umabot  1-2 araw o higit pa.  

  Agad naman na ipapa alam sa iyo  kung ito ay handa na para ma pick-up sa  aming opisina sa pamamagitan ng iyong email.

DETALYE (ilagay ang mga impormasyon na kailangan. I click lang ang attach files at puwede mo nang picturan ang mga requirements gaya ng ID, Reseta, Authorization Letter at Clinical Abstract galing sa doctor gamit ang iyong cellphone). *IMPORTANTE PO NA MA-ISAMA LAHAT NG KAILANGAN NA REQUIREMENTS SA IYONG MGA ATTACHMENTS tulad ng ID, Reseta, Authorization Letter (kung hindi ang pasyente ang may request), at Clinical Abstract galing sa Doktor.
